Most of us are familiar with the bright pink POLST form, a unique tool that provides a portable medical order honored across a variety of settings in California. Some of us were involved in helping the original POLST legislation become law in 2009 and many others may recall SFEOL training sessions when it was new. Since then, much progress has been made on its adoption but more remains to be done.

Our own Connie Borden will engage in conversation with Jennifer Moore Ballentine, MA, Chief Executive Officer of the Coalition for Compassionate Care in California (CCCC). The coalition addresses a broad array of end-of-life issues. In the POLST arena, it provides resources for patients and providers, develops POLST policy and monitors the form’s effectiveness.
